What do I need to bring to my appointment?

As a patient of McGovern Physical Therapy Associates, you should bring the following to your first visit:

  1. All of your insurance information, including your insurance card.
  2. If your case is a third party liability we will need the name of the insurance company, phone number, adjuster and claim number.
  3. After the physical therapy examination, the therapist will develop a plan of care that meets your individualized goals. They will establish how often and for how long physical therapy will be needed. This is a collaborative process and your input will be integrated into the plan.
  4. The prescription from your doctor.
  5. The filled out intake forms that you have downloaded from this site.
  6. Loose and comfortable clothing.
  7. If you are a minor, you need to bring a parent or have a parent’s signature on our consent form.
  8. Bring your calendar so we can schedule future visits.
What should I expect on my first visit?

As a patient of McGovern Physical Therapy Associates, you can expect the following at your first visit:

  1. You should anticipate that the first visit with the physical therapist may last up to one hour.
  2. A thorough evaluation and explanation of your particular diagnosis and problems.
  3. After the physical therapy examination, the therapist will develop a plan of care to meet your individualized goals and establish how often and for how long physical therapy may be indicated. This is a collaborative process and your input will be integrated into the plan.
  4. Expect to be asked about your personal goals for recovery. We expect you to ask us questions too. Communication is vital in your recovery.
What can you expect from us?

As a patient of McGovern Physical Therapy Associates, you can expect the following:

  1. to be treated with courtesy and respect
  2. to be seen in a timely manner
  3. to have all treatments explained to you until you have full understanding
  4. to be given care with a systematic and reliable approach to achieve the best outcome
  5. to refer you to a specialist if your condition does not improve as expected
  6. to help you recover from your injury and get you back to your life
  7. to keep your records and condition in strict confidence.
What we expect from you?

As a patient of McGovern Physical Therapy Associates, we expect the following from you:

  1. to be cooperative and follow your Plan of Care as prescribed by your therapist
  2. to give us the courtesy of a phone call if you cannot make your appointment
  3. to participate to the best of your ability in each therapy session
  4. to comply with your therapist’s treatment recommendations, including all exercises
  5. to comply with your exercises and home treatments as prescribed by your therapist
  6. to report to your therapist any and all changes in your condition.
What can you expect about your recovery?

As a patient of McGovern Physical Therapy Associates, you can expect the following about your recovery:

  1. the body has a healing process of its own that is accelerated with physical therapy, but healing is not instantaneous
  2. following your Plan of Care and your therapist’s instructions will speed your healing process and give you the best outcome
  3. you may experience some discomfort after each session, which is normal when treating injured areas; please follow your home instructions for treatment to curb discomfort
  4. the less you do at home, the longer your recovery will take
  5. set backs in your recovery may occur and they are normal; perseverance with your treatment will get you back on track.
